When a Pharmacy accepts participation in the program; then any and all contact will be with Personnel from the local Food Bank ONLY

The judgement of the Pharmacy-Pharmacist for the determination of the Vitamins and Supplements is final.
PHARMACY & FOOD BANK

The Pharmacy/ Pharmacist is the “Key Person” in sustaining and maintaining the smooth flow of the program

The Pharmacist receives information regarding the children in the 2 age groups (0 to 1-year and 2-12-years) with additional information for special needs from the Food Bank.

The Pharmacist dispenses the Vitamins and Supplements according to the information provided and submits the shared cost to the Food Bank Personnel.

Appropriate invoices and receipts are maintained by the Food Bank and the Pharmacy
PHARMACY & THE FAMILY THAT DOES NOT ACCESS THE FOOD BANK

When a family, that does not access the Food Bank, approaches a Pharmacy to join the VITAMIN SUPPORT PROGRAM

The Pharmacist may contact an Advocate to locate a Sponsor or if an Advocate is not identified the Pharmacist may contact the local Food Bank to initiate the program for the family.

The cost of the vitamins will be shared by the Pharmacy and the Sponsor or the Food Bank – depending on the arrangement.
